A referral process designed around you
Referring a patient for specialised rehabilitation is simple. You can download and complete the Patient Referral Form on our referral page, or message one of our dedicated case managers directly on WhatsApp. From there, our team takes over: handling the clinical assessment, confirming bed availability and arranging medical scheme authorisation, and keeping you updated throughout the process.

1. Download and complete the Patient Referral Form (form IDH020) from the Intercare referral page, then send it back to the relevant case manager.
2. Message a dedicated case manager directly on WhatsApp to start the conversation and share clinical details.
What happens after you refer
Once we receive your referral, our interdisciplinary team coordinates everything needed for a smooth admission. This includes a clinical assessment to confirm the patient's rehabilitation goals and needs, checking bed availability at the most appropriate facility, and obtaining the necessary medical scheme authorisation. You are kept informed at each step, so you always know where your patient is in the process.
